Our Care Coordination & Advocacy Program is built to ensure no client ever feels alone in their recovery journey. We act as both a guide and an advocate, working alongside clients and families to make sure their needs are met.
1. Resource Connection
We help clients identify and access critical resources such as:
Housing support and placement assistance.
Education programs, including GED preparation, vocational training, and higher education opportunities.
Employment resources, including job readiness training, career counseling, and placement assistance.
Care, ensuring access to primary care, specialists, and ongoing treatment.
